體外心臟震波治療對豬急性心肌梗死后內(nèi)皮型一氧化氮合酶及堿性成纖維細胞生長因子表達的影響

摘要: 目的觀察體外心臟震波(CSWT)治療對豬急性心肌梗死(AMI)后血清及心肌組織中內(nèi)皮型一氧化氮合酶(eNOS)及心肌組織中堿性成纖維細胞生長因子(bFGF)含量的影響。方法制造豬AMI模型,把12只豬隨機分為2組,每組6只。以單純心肌梗死為對照組,實驗組接受CSWT治療(于術(shù)后1、3、5 d共治療3次,震擊:200次/點,共12點,能量為0.09 mJ/mm~2)。分別于手術(shù)前,以及手術(shù)后8個不同時間點(術(shù)后選即刻、1、3、5 d,1、2、3、4周時)抽取外周血,用酶聯(lián)免疫反應(yīng)方法檢測血清eNOS含量。1個月后處死動物,心肌組織取材,半定量RT-PCR法檢測心肌組織中eNOS及bFGF表達量。結(jié)果實驗組術(shù)后1 d CSWT治療后即有eNOS表達升高,5 d達高峰。對照組eNOS隨時間推移逐漸減少。兩組有明顯差異(P<0.01)。半定量RT-PCR檢測梗死交界區(qū)心肌組織實驗組eNOS及bFGF表達均明顯高于對照組[eNOS:(27.705±4.13) vs (16.448±3.21);bFGF:(32.571±4.23) vs (17.858±4.17),P<0.01]。結(jié)論 CSWT治療AMI能夠有效促進eNOS及bFGF表達升高,該方法可能成為治療心肌梗死的一種新手段。 Objective To observe the effects of extracorporeal cardiac shock wave therapy(CSWT)on cntent of endothelial nitric oxide synthase (eNOS) and basic fibroblast growth factor (bFGF) in serum and myocardial tissue in swine with acute myocardial infarction (AMI).Methods 12 Model swines with acute myocardial infarction was set up and were randomly divided into two groups with six in each group:simplex myocardial infarction as the control group and the experimental group which received CSWT treatment(three times on the first,third,fifth days after operation. 200 hit/point.Total:12 points.Energy:0.09 mJ/mm~2).Peripheral blood was extrated at eight different time points before and after operation (immediate,the frist,third,fifth day,1st,2nd,3rd and 4th week after operation) to detect serum eNOS content with enzymeliked immunosorbent assay.Materials at myocardial tissue from the swines killed one month later were obtrained to detect expression amount of eNOS and bFGF with semi quantitative RT—PCR method. Results eNOS rose up in the experimental group one day after CSWT and reached the peak on the fifth day,whereas eNOS gradully reduced in the control group,which slowed significant difference between the two groups (P<0.01). Detection with semi quantitive RT-PCR of myocardial tissues of infarction border area showed that eNOS and bFGF expression in the experimental group were obviously higher than those in the control group [eNOS(27.705±4.13)vs (16.448±3.21),bFGF:(32.571±4.23) vs (17.858±4.17),P<0.01)].Conclusions Extracorporeal cardiac shock wave therapy of acute myocardial infarction can effectively promote rise of eNOS and bFGF,which may be a new way to cure AMI.
