手部深度燒傷后瘢痕攣縮畸形的治療

摘要： 目的:探討手部深度燒傷后瘢痕攣縮畸形的治療方法。方法:本組58例極76只手部燒傷后瘢痕攣縮畸形患者,采用瘢痕切開或切除,嚴(yán)重者行關(guān)節(jié)融合或成形術(shù),局部皮瓣加皮片移植,或腹部皮瓣移植;術(shù)后早期進(jìn)行手部康復(fù)治療。結(jié)果:移植皮片和皮瓣均成活,手指指間關(guān)節(jié)和掌指關(guān)節(jié)功能、指蹼功能均良好,生活能夠完全或大部分自理。結(jié)論:對(duì)手部深度燒傷后瘢痕攣縮畸形采用手術(shù)和功能康復(fù)治療相結(jié)合的方法可取得較好的效果。
Objective:To explore the method of treating scar contracture deformity caused by deep burn in hands. Method:58 cases(76 hands) with scar contracture deformity caused by deep burn in hands were treated by scar incision or excision.In severe cases,joint fusion or arthroplasty was made,local flap plus skin grafting or abdominal flap grafting also were conducted;early hand rehabilitation treatment was required after operation.Result:Skin and flap grafting were all survived.Function of interphalangeal joint and metacarpophalangeal and finger web were all good;patients took care themselves life daily in full or most time.Conclusion:Better effect can be obtained when the patients with scar contracture deformity caused by hand deep burn are treated with surgery combining with early rehabilitation treatment.
